Passport Information

We accept Passport Applications from 8am until 3:30pm daily, Mon – Fri unless closed for a Holiday.

The DS-11 passport application can be downloaded from www.travel.state.gov or acquired through our office at the Grand Forks County Office Building at 151 South 4th Street, Grand Forks, ND phone: 701-780-8251

Fees: We cannot take cash, debit or credit cards for payment to Dept of State. Payment to them should be personal check, money order or cashiers check.

For those 16 and over
(1) $110.00 Passport Book or $30.00 for Passport Card

For children under the age of 16:
(2) $80.00 Passport Book OR $15.00 for Passport Card

Payments to Grand Forks County Equalization can be made by Personal check, money order, cashiers check, cash, debit or credit card.

(3) $ 25.00 – Processing Fee Payable by Check or Cash To:
GRAND FORKS COUNTY TAX EQUALIZATION
(APPLIES TO ALL APPLICATIONS)

Forms Required for application process: sent in with application and returned to you
(1) State certified copy of Birth Certificate
(2) Naturalization Papers; or
(3) Most recent passport - this must be submitted - if lost or stolen must complete a lost or stolen form – (DS-64)

Photos
One color passport photo - photo must have been taken within the last 6 months, it must be passport quality.

Passport photos can be taken in the Tax Equalization Department for a fee of $13, or at any of the local photo studios. check your local phone book for listings

Proof of Identity
A current and valid drivers license or military id is required. If out of state (not ND) drivers license - A second ID will be required.

Effective Feb 1st, 2008, as provided by case of federal regulations title 22, section 51.28 , passport applications for children under 16 years requires both parents signatures. See www.travel.state.gov for requirements

All children must be present and must be accompanied by both parents if under 16 or one parent if 16 to 18

***Do not sign application. Signature must be witnessed by a Passport Agent***

Expedited Passports

Passports can be expedited by including the extra $60 expedite fee with the regular fee. In some cases, (if needing the passport within 10 days or less) proof of travel is required. At this point, it is recommended the customer make an appointment with the Minneapolis office and appear in person to expedite the passport process.

If the customer would like the passport returned by Express Mail, they include an amount of $20.66 with the payment to Passport Services (Dept. of State).
